Are you dealing with a nagging discomfort in between your shoulder blade and your spine? No matter how many stretches you do, or how much time you spend foam rolling it, does it still persist? Pain in this area often gets labeled as a rhomboid issue because it is a relatively superficial muscle that you can appreciate from any anatomy chart. However, how to fix rhomboid pain sometimes may actually have little to do with the rhomboids. In this video, you'll learn the ultimate secret of how to fix rhomboid pain by taking a look at a very specific movement...thoracic rotation! Rhomboid pain is labeled as pain in between your shoulder blade and your spine (interscapular pain) that can even refer up to the base of your neck or down the middle of your back. It is often described as a nagging dull ache or pain, pressure, knifelike, pulling, or even a burning sensation (is that really rhomboid pain? Food for thought). You'll often find people dealing with discomfort in this area wanting to fidget around in efforts to feel better - moving their head and neck around trying to stretch, rounding their shoulders, squeezing their shoulder blades back, or twisting their upper back, you name it. The occasional soreness in this region is nothing to be concerned about, especially if you did a hard upper back workout or carried a backpack for a long time. However, dealing with constant discomfort or pressure in this area can become a real nuisance. Just because you're dealing with discomfort in between your shoulder blade and your spine doesn't mean we have to point the finger at the rhomboids every time! To fix rhomboid pain, we need to take a look at and consider all of the other anatomical structures in the area. If there was only one movement assessment I had to pick when it comes to evaluating interscapular pain, I'm going to look at thoracic rotation mobility. Poor thoracic rotation mobility can wreak havoc on the body and can definitely refer pain and discomfort to the rhomboid region. Poor thoracic rotation mobility is often coupled with poor scapular mobility/stability, which only contributes to the issue more! I have found that simply improving thoracic rotation mobility can improve interscapular discomfort. We sat down with Dr. Claudia to see what's the deal with several common period pain relief methods. A hot water bottle or heat pack for period cramps may be old school, but it's stuck around for a reason. Heat really can help with relief. When it comes to pain killers, you need to keep in mind any other medications you're taking as well as your personal reaction to the medicine. Exercise may have mixed reviews, but there's some evidence that it can help relieve your cramps. Just make sure to listen to your body: if it hurts worse, skip the movement until you feel better. Birth control can also help relieve particularly bad period symptoms. It is the characteristic presentation of some sort of irritation to the sciatic nerve. WHAT CAUSES SCIATICA These sensations of numbness, tingling, or pain are the result of an irritation to the sciatic nerve. Whether it’s a disc in your back, your vertebrae, or a muscle - anything that impinges and puts pressure on the sciatic nerve can result in sciatica nerve symptoms. BEST EXERCISE TO TREAT SCIATICA Now depending on the exact cause of your sciatica, the treatment exercise progression looks a little different. For example there are certain exercises I would give to a patient with sciatica from a bulged disc that I absolutely would NOT recommend for someone with sciatica from stenosis or degenerative disc disease (it’s crucial that you check with a local healthcare provider for exercises specific to your unique diagnosis). While these various pathologies are all treated a little differently, today I wanted to demonstrate for you the one exercise I give the majority of my patients regardless of the cause of their sciatic nerve pain. THE BEST EXERCISE FOR SCIATICA NERVE PAIN Think of your nerve as a “string” (or “floss) from your back to your big toe. Somewhere along the course of this nerve, something is “pinching” the nerve and causing pain (or numbness or tingling) sensations. What we need to do is “stretch” or “floss” this nerve back and forth to free it up from this “pinching” and decrease the painful nerve signals. This is best accomplished with “nerve glides” or “nerve flossing”. HOW TO PERFORM NERVE FLOSSING FOR SCIATIC NERVE PAIN 0:00 INTRO 0:54 LEG EXTENSION 2:28 ANKLE PUMPS 3:30 BULGED DISC BONUS 5:21 STENOSIS BONUS 6:29 PIRIFORMIS BONUS 7:28 DON’T MISS THIS BONUS SCIATICA HELP As I mentioned above, it’s crucial that you perform the right exercises for the specific sciatica diagnosis you’re dealing with. Counting down the four WORST positions to sleep in with a painful shoulder, and what you should be doing instead! Fix your sleeping position to minimize your shoulder pain and sleep better through the night. Simple tips from a physical therapist to help you get a more restful, pain-free night’s sleep and experience less shoulder pain when you wake up. The only thing worse than shoulder pain is shoulder pain that wakes you up in the middle of the night. We all know that sleep is important for healing; it’s the body’s time to recharge, recover, and repair. But when your sleep gets interrupted by the very thing it’s trying to heal… well… that’s a recipe for disaster. Most often the shoulder pain that you’re experiencing in the night or in the morning is due to your sleeping position with that shoulder. Most people unknowingly put too much pressure on it or hold it in such a position that stress is placed on already-impaired structures. If you suffer from shoulder pain that is waking you up at night or hurts so bad you can barely move it in the morning DO NOT despair! One of the best things you can do for it is to make some simple adjustments to HOW you’re sleeping to get a better night’s rest free from shoulder pain. In this video I’m sharing four simple tips to eliminate your nighttime shoulder pain. These are different fixes for your sleeping positions to decrease stress on your shoulder and help you to sleep better. I’ll present this in a “Top Four” format - we’ll countdown the four worst sleeping positions for your shoulder and I’ll give you some simple tips on what you can do instead to make for a more restful, comfortable night. WORST SLEEP POSITIONS FOR SHOULDER PAIN - AND WHAT TO DO INSTEAD! 4. ON YOUR BACK WITH YOUR HANDS BEHIND YOUR HEAD - it’s better to bring that arm down to your side, but even then it’s still in an extended position and can put stress on the front of your shoulder. Instead lay on back and supported under your your elbow 3. ON YOUR NON-PAINFUL SIDE, INJURED SIDE UP - Grab two pillows to hold it up (2 pillows better than one). Prop one under your elbow and the other in front of your body. 2. ON YOUR PAINFUL SIDE, INJURED SIDE DOWN - Best thing to do is unload it. Grab two pillows and put one about 6 inches below the other. This creates a canal to unload the shoulder as you have one supporting your torso and the other supporting your head/neck. 1. ON YOUR STOMACH WITH YOUR ARMS UNDER YOUR HEAD Honestly the fix here is just don’t do it. If you want to fix your shoulder pain, neck pain, upper back pain, and lower back pain - you need to stop sleeping on your stomach. I hope those tips help you to sleep better with your shoulder pain. I give those to my physical therapy patients regularly and I know they can be super effective. I hope you find them helpful in alleviating your pain as well. Strengthen and stabilize your knee joint with 5 simple exercises for your quads, hamstrings, and hips. = Be sure to supplement these strengthening exercises with the right stretches for your knees! This 7-minute follow-along stretching routine is what I recommend: 🤍 = JARED'S RECOMMENDED FOAM ROLLER (aff): 🤍 JARED'S EXTRA THICK HOME WORKOUT MAT (aff): 🤍 = Increasing the strength and function of the muscles that act on the knee joint is a great way to decrease pain, and improve function. Now when we talk about the mechanics of the knee, it’s a relatively simple joint. It flexes (bends) and extends (straightens) and that’s about it. The muscles that are primarily responsible for these two actions include the quadriceps, hamstrings, and one of your calf muscles. Increasing strength and function in these muscles has been proven to have a positive effect on decreasing knee pain. However, as a doctor of physical therapy and a trained movement specialist, we are taught to find the CAUSE of pain, not just the location. Often times knee pain is simply a manifestation of faulty mechanics either higher or lower in the leg. It’s actually your hip and ankle that primarily determine where your knee joint bends and straightens and therefore play a bigger role in the mechanics at the joint. Improving strength and stability in these areas is vital in knee rehab and decreasing pain in the knee joint. Presented in this video are the best exercises to increase strength of the muscles that flex and extend your knee as well as those that play a key factor in knee joint mechanics and function. (2:42) QUAD SET: a simple, sustained contraction to the quadriceps muscle to increase strength and voluntary firing. (3:43) STRAIGHT LEG RAISE: adding hip flexion to the quad contraction is an effective way to strengthen the knee. (4:55) SHORT ARC QUAD: adding slight flexion and contracting the quad into terminal extension can be an effective means of strengthening as well as getting the knee joint used to moving again. (5:59) BRIDGES: Great way to increase glute and hamstring strength without introducing potentially painful motion at the knee. I’m actually going to provide an in-depth demonstration of this technique so you can get relief right at home.  Everyone always focuses on working on the muscles on the back of the neck. No one ever works on the muscles on the front part of the neck, and that’s what you really need to do. You can have someone demonstrate this neck pain technique on you, or you can do it yourself.  This unique neck pain stretch is especially good if you’ve had a whiplash injury or if you have stiff neck problems. Upper neck pain could also come from a digestive issue.  SPECIAL NOTE: When you do this stretch, make sure you keep your hand stationary on your neck—do not rub up. There are 2 arteries called the carotids that run parallel to your neck muscles, and you do not want to close them off and feel dizzy.
